FAMILY ENGAGEMENT SURVEY
Each year, the Massachusetts Department of Elementary and Secondary Education (ESE) surveys families of students with an IEP in selected school districts to determine how well your child’s school is communicating with you and promoting your engagement in your child’s education. It’s your district’s turn this year. The web-based survey will be open from September 1, 2016 – June 30, 2017.

- You can also complete a paper survey and mail it in or ask you district to mail it to the address below.
- Ask your child’s teacher if you need help accessing and completing the survey.
- Your participation in this survey will help your school district to improve family engagement for all students.
Who completes the Family Engagement Survey? Parents or guardians of students with an IEP who attend school are asked to complete a survey. Districts are asked to participate in this survey once every four years. This year is your district’s turn to participate.
I have more than one child with an IEP. Should I complete a survey for each child? Yes, you should complete one survey for each child. Please make sure you complete only one survey for each child with and IEP.
What should I do if I need help in finding and completing the survey? You can ask your child’s teacher to help you find and complete the survey. Schools are finding ways to help all families participate in the survey.
Is the survey confidential? The survey is completely confidential and won’t include your name or your child’s name. There is no personally identifiable information requested on the survey.
